Monthly Report of Volunteer Hours

We so appreciate the time and effort that goes into volunteering on a care team for The Care Communities. We could not offer these services without the generous gift of your time - you are the backbone of our organization. 

In an effort to keep up with reporting requirement for Grantors and other organizations that we will continue to receive funds from, it is vitally important that each volunteer report ALL their hours spent in the process of working with a care partner.  The total number of hours reported per month should include all time spent with your care partner, including travel time and all communication time with your care team and our staff.
